Self-pay average across telehealth providers. Median $249/mo, range $205-$339/mo (n=50 states).
Averages reflect self-pay (no insurance) monthly costs across telehealth providers shipping to each state, plus retail pharmacy benchmarks. Real out-of-pocket varies by drug (Wegovy ~$499/mo on NovoCare, Zepbound $349-$499/mo on LillyDirect), insurance, and provider choice. Data scan: May 2026.
Tiers reflect the national distribution of self-pay averages. States in the low tier tend to have heavy compounded-provider activity; high-tier states reflect more retail pharmacy mix and stricter telehealth rules.

The same Wegovy or Zepbound prescription can cost dramatically different amounts depending on where you live — even with the same drug, same dose, and same manufacturer programs. Three factors drive the difference:
About 14 states require synchronous video consultation for new GLP-1 prescriptions; the rest permit asynchronous (chat / form-based) telehealth. Video-only states tend to have higher costs because fewer providers serve them and the consultation fee is higher.
States with comprehensive Medicaid GLP-1 coverage (California, Massachusetts, Pennsylvania, Virginia, Minnesota) generally see slightly higher self-pay averages — because the cheapest segment of the market is absorbed by Medicaid. States without Medicaid coverage (Arkansas, Mississippi, Idaho, Missouri) push more demand into the cheapest cash-pay tier, pulling the average down.
States with high obesity rates and lower household income (the Deep South, parts of Appalachia) have more active compounded-pharmacy advertising. Compounded semaglutide and tirzepatide cost $199-$349/mo — far below NovoCare ($499) or LillyDirect ($349-$499). When this segment dominates a state's mix, the average drops. Note: compounded GLP-1 are NOT FDA-approved as finished products. See our compounded GLP-1 buyer guide for safety considerations.
